This app name is Hidden Eye. It is so easy to use.
How to install and use it :
1. First of all download this app from here.
2. Then install it in your phone as like as regular installation systems.
3. Then open it. When you open it you will see like this image.
5. Then active " Activate device administrator ". Like this image.
Now you can test it yourself. First type wrong password and then type right password. When you unlock your phone then phone notify you that who did try to unlock this phone with your picture.
